Process for Obtaining Death and Birth Certificates from the Medical Record Section

Death Certificate

Required Documents:

  1. Copy of identity proof of the applicant (legal heir of the deceased).
  2. Copy of identity proof of the deceased.
  3. Copy of Death Intimation received from AIIMS.
  4. In case of MLC death, copy of the post-mortem report with an authentic stamp from the police station.

Application Process for Death Certificate (AIIMS Bilaspur):

  1. Application form for Death Certificate (to be filled by the legal heir).
  2. Copy of applicant's identity proof (such as Aadhaar).
  3. Copy of identity proof of the deceased.
  4. Copy of Death Intimation received from AIIMS.
Application Process for Death Certificate (AIIMS Bilaspur)
  1. Post-mortem report signed by the doctor/police station with an authentic stamp.
  2. Copy of identity proof of the applicant (legal heir only).
  3. Copy of identity proof of the deceased.

Instructions for Obtaining Death Certificate in MLC Death Cases:

  1. Submit all the above documents to MRD.
  2. The applicant must fill Death Report (Form No. 2) as the informant.
  3. Submit the application form along with all documents to the MS Office.
  4. All documents signed by the MS will be forwarded to MRD.

Birth Certificate

Required Documents:

  1. Copy of father's identity proof.
  2. Copy of mother's identity proof.
  3. Copy of the newborn baby's discharge summary received from AIIMS.

Steps for Applying for Birth Certificate (AIIMS Bilaspur):

  1. Application form for Birth Certificate (to be filled by the mother or father only).
  2. Copy of father's Aadhaar Card.
  3. Copy of mother's Aadhaar Card.
  4. Copy of discharge document issued by AIIMS.
